A Note About File Formats

Many of the files here were created using Adobe Pagemaker (PMD files). When opening some of these files, a dialog box may appear saying it could not find certain fonts, and asking which font you would like to use to replace the missing fonts. Because of the EULAs (End User Licence Agreement) that accompanied the fonts used we are not permitted to make these fonts available for download, so please do not ask for them. Simply use a different font, or you can search the web and buy (or download for free in some cases) the needed fonts. View the PDF files to see how the fonts are "supposed" to look. We encourage you personalize these materials by using your own fonts and graphics. Also when opening the PMD files, the program may say it cannot find image files that were linked to the document. Simply click "Ignore" in this case as the graphics are embedded in the document.

These files are also available in Adobe Acrobat format (PDF files). If you have either of these programs, you should be able to edit either format of the files. If you do not, you should be able to view the Acrobat files using the free Adobe Reader program and use the files as a guide for creating your own forms.

Graphic files are available as high resolution TIF files. Most word processing and graphic programs are able to import TIF files.

For all PMD, PDF, TIF, and MS Word files below, right-click on the link and select "Save Target As..." to save the file to your computer.
